WHAT H4M CAN DO FOR YOU:
Find doctors, dentist, and mental health specialists that see you in a timely manner.
Help you get affordable perscription medicines.
Provide health screenings at festivals and other music events.
Present health seminars that are specific to musicians' health issues.
Connect you with other beneficial community programs
MUSICIANS CAN CALL US AT 337-988-1583:
We take your information over the phone and verify that you are a working musician.
We give you clinic options.
Then you decide which clinic to visit and make your own appointment. We let you know what information you will need to take to your first visit
WHO IS ELIGIBLE:
Musicians who make at least 1/3 of their income from performing are eligible for our free services.
Musicians who live in Acadia, Allen, Beauregard, Calcasieu, Cameron, Jeff Davis, Evangeline, Iberia, Lafayette, St.Landry, St. Martin, St. Mary, and Vermilion parishes.
Retired musicians who met eligibility requirements while they were performing are also eligible.
WHAT IT COSTS:
H4M referral services are free.
Call today and receive information and get registered with the program.
Then, if a musician chooses to visit a doctor, at one of our participating clinics, they will be charged based on their income and ability to pay.
If they need specialized treatment and testing, a referral will be sent to LSU/UMC, an area specialist (if available), or the New Orleans Musicians Clinic. The charge for this service will also be based on their income and ability to pay.
